After-Care provides disability support services to people who receive Individualised Funding from Disability Services Victoria.
People with a disability and their families increasingly have more say about which services they choose to use. If you receive Individualised Support Program (ISP) Respite for Older Carers (ROC) or individualised Futures for Young Adults (FFYA) funding, you or your family can decide where to use the funding to get the ‘best value’ supports.
After-Care takes the time to get to know you, building a trusting, long-term relationship with our Carers and Coordinators. We carefully match supports to meet your specific needs, so that you maintain well-being and stay in control of your life.
Whether you receive direct payments, or use a financial intermediary such as Moira FI, After-Care provides cost effective transport, in home respite, social support and personal care without the hassle of managing your own employees.
Using After-Care allows you to focus on living your life to the full while we take care of recruitment, training, rostering (including last minute changes) worker safety and payroll.
If you have case management with Department of Human Services, Victoria, you can still choose After-Care for your support. We have an ISO accredited Quality Management System to ensure our services are provided to the highest possible standards.
For people managing their own package, we are able to provide an extra level of ongoing support coordination or occasional assistance from a case manager should your circumstances change. You can also access our ‘after hours’ emergency service that can assist you and your family in a crisis.
