If you’re thinking about switching to After-Care, we’re here to make the process easy and stress-free. From start to finish, we’ll guide you every step of the way – because quality care should always feel simple and supportive.
How to switch
Review your current agreement
🔹 Check your Home Care Package agreement for any exit conditions or notice periods.
🔹 Look out for any exit fees (these should be listed in your agreement).
Find a new provider
🔹 Research providers that better suit your needs.
🔹 Consider factors like services offered, flexibility, fees, and reputation.
🔹 Reach out to potential providers and discuss how they can support you.
Notify your current provider
🔹 Inform your current provider in writing that you’re switching (following their required notice period).
🔹 Request a final statement of funds to see what’s left in your Home Care Package budget.
Contact My Aged Care
🔹 Call My Aged Care (1800 200 422) and ask them to unassign your Home Care Package.
🔹 Let them know the details of your new provider so they can transfer your package funds.
Sign up with your new provider
🔹 Your new provider (like After-Care 💙) will help set up a new care plan tailored to you.
🔹 They’ll work with My Aged Care to ensure a smooth transition, with no gap in services.
